Somewhere between a doctor's office and the gray market sits the medical spa, "wellness clinic," or concierge practice. These are businesses that administer or prescribe peptides as part of an injectable "anti-aging" or "peptide therapy" program. Think of them as the middleman — and the middleman charges for the convenience.

What the markup looks like

Med spa pricing is frequently the most expensive public-facing route. It's common to see peptide programs quoted at $400–$600 or more per month, and in some cases well beyond that. The clinician visit, the injection administration, the "protocol," the monitoring, and the spa's overhead all get folded into the price.

Here's the part that surprises people: in many cases the underlying product the spa is using is itself sourced from research-use (RUO) suppliers — the same gray market we'll cover next. So you can pay a large markup for a product that came from the same place a $60 vial came from, wrapped in a nicer experience and a clinical veneer.

Not all clinics are the same.Some are legitimate medical practices using FDA-approved products through compounding or prescription channels with real oversight. Others are marketing-first businesses. The price tag alone doesn't tell you which is which — you have to ask what's actually in the vial and where it came from.

What to watch for

  • Whether they can tell you the exact supplier, batch, and COA of the product
  • Whether the "protocol" is personalized or a one-size-fits-all upsell
  • Pressure to sign up for long, expensive packages
  • Vague answers about what compound you're actually getting and its source

The honest framing: med spas sell convenience and supervision. For someone who values the hand-holding and monitoring, that markup can be worth it. For someone who understands the underlying product and wants to control cost, the same material is often available far cheaper — which is where the gray market enters the picture.
